In 1830, the Blessed Virgin Mary appeared in Paris to a Visitation nun, Sister Catherine Laboure (who is now a Saint) and showed her a vision of a medal that she wished to have made. The vision was just as you see it on our Miraculous Medals, with rays (representing graces) flowing down to the earth from her outstretched hands, with an inscription surrounding, saying "O Mary Conceived Without Sin, Pray For Us Who Have Recourse To Thee."
On the reverse, the vision specified an "M" surmounted by a cross, with two hearts (of Jesus and Mary) and 12 stars.
